Since you're using WordPress I suggest that you search for the appropriate WP plugin which creates such form. If this plugin uses Widgets you can add Widget area control to the Header where you will be able to add widget with form in WordPress >> Widgets.
The Widget Area control in available in Themler under the Insert tab.
